If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please call our office if you have any questions at all.

 CANCELLATION POLICY

At Dr. Sheila Sales office, we consider the time aside for your dental appointment  to be yours alone. Consequently, patients that cancel for appointments, especially at the last minute, and patients who do not appear for their appointments, affect our entire practice.

 We understand that cancellations are sometimes necessary, but we all pay the price for last-minute cancellations. Plus, when you routinely miss your appointments, your dental health suffers as well. 

Not only are we committed to bringing you the very best professional and personal care that we can, we also place value on your time. We expect the same respect by giving us enough advanced notice when you cancel an appointment so that we can use that time for the benefit of our other patients.

I understand that a fee of $75 for all missed/cancelled appointments without a 24 hr. notice will apply.  (Monday appointments must be cancelled Thursday before 2pm)
